This section of the Governor's Volunteer Awards Web site has been developed exclusively for the agency coordinators who help make the program successful.  Thank you for your assistance!

Submit nomination form to state agency coordinator.

Submit nomination data via the Internet.  Include fund, agency and org. numbers for recognition costs.

Send congratulatory letter to award recipients.

Make address corrections in database.  New nominations will not be accepted.

Mail invitation from Governor's Office inviting recipients to attend award ceremony.

Documents to Download — Depending on your Internet browser's configuration, you may need to right-click on the link and “Save As..” to your computer.  Most of the files available to download are provided in pdf format.

Nomination Form (download: [MS Word: 168k]) — VERY IMPORTANT INFORMATION!

  1. Coordinators must customize the nomination form before distributing it (whether by hard copy, electronically, or both).
  2. DO NOT distribute the form without Page 1 overview and instructions.
  3. Forms should be distributed by February 2 and are due back to you by March 9.
  4. If you would like your customized form to be available from the official GVA Web site, submit an electronic copy (or a link to your Web site) to jody.benz@iowa.gov.

Database Download Instructions [MS Word: 11k] or [PDF: 6k]— You may download your agency's records if you wish to keep an electronic record of your own.  The records will also be stored permanently on the awards Web site.
